- School’s address (if student taking up another degree in another school)

- For College of Engineering alumni: If you are a member of other Engineering-based organizations, please indicate what those organizations are, and choose one org (either UP ACM or another org) you will be affiliated in. You need to affiliate in only one org for purposes of UPAE Grand Alumni Homecoming.
